Enclose
with the sample(s) a completed copy of our Sample
Submission Form, or a
letter that clearly states:
• Name of person to whom results should be reported
• Complete Mailing Address
• Telephone Number and/or Fax Number
• Purchase Order Number (PO#) or Visa/MasterCard # with
Expiration Date
• Sample Identification (ID) as it should be recorded on
Final Report • Test/Catalog Number of test procedure (see Services
Page)
Note: Each sample vessel must have a label; and, the label must
associate the sample to the sample identification recorded on the enclosed
submission form. This is required for compliance with Current Good Manufacturing
Practices (CGMP) for Test/Catalog #: M-200, M-250, M-300, M-350, M-700
and M-800.
Please
call Technical Services at 518-891-2356 (or Fax the sample submission
form to 518-891-5753) no later than 12:00 pm ET on the day you ship samples. Please
report the number of samples shipped and the specific mycoplasma test/catalog
# requested. This will help Bionique® Testing Laboratories meet
your need for quality, cost-effective and timely service.

and does
not accept test materials that exceed BSL 2 handling requirements.
Bionique ®Testing Laboratories does
not accept test materials containing HIV, Herpes viruses,
or BSL
3 or 4 agents. Bionique® Testing Laboratories
reserves the right to determine the acceptance of certain test materials. Please
call Technical Services, at 518-891-2356, to verify acceptance of
your sample, before shipping any sample containing human
and animal viruses, potential human or animal pathogens,
or other biohazardous
contents.

Sample Preparation: Bionique® Testing Laboratories recommends
freezing samples without cryo-preservatives and storing samples at or below – 60 oC and shipping
frozen samples on sufficient dry ice to maintain optimal sample integrity for optimal detection of mycoplasma. Note: Not Applicable to M-100 and M-175 Cellshipper®tests

Prepare your samples per specific test instructions. Remember the accuracy of our test depends on the quality of your sample.
Send sufficient volume of sample in an appropriately sized container for test requested. Remember that the volume of aqueous solutions (e.g. media, cell cultures, sera, etc.) changes in relation to temperature and air pressure.
Package your sample to maintain stable conditions and prevent breakage.
Ambient shipments should be packaged to protect samples from leakage, freezing and/or overheating, crushing and/or impact. Samples must be received at Bionique® Testing Laboratories intact with no evidence of leakage or other sample compromise in order to be accepted for testing.
Frozen shipments should be packaged with sufficient dry ice to keep the enclosed samples frozen for a minimum of 2 days. Samples shipped frozen must be received frozen with dry ice remaining to be accepted for testing at Bionique® Testing Laboratories.
Ship sample(s) next day courier service for receipt at Bionique® Testing Laboratories, Monday through Friday, 8:15 am through 3:45 pm Eastern Time. Note: Please refer to specific courier service guides to determine which guaranteed delivery services are provided to this location.
